چکیده

Ischemic stroke is a rare event in childhood. In approximately one third of cases no obvious underlying cause or disorder can be detected. We investigated the importance of genetic risk factors of venous thromboembolism in childhood or stroke in adulthood as risk factors for spontaneous ischemic stroke in children. One hundred forty-eight Caucasian infants and children (aged 0.5 to 16 years) with stroke and 296 age-matched controls from the same geographic areas as the patients were analyzed for increased lipoprotein (a) [Lp(a)] levels G30 mg/dL; for the presence of the factor V (FV) G1691A mutation, the prothrombin (PT) G20210A variant, and the TT677 genotype of methylenetetrahydrofolate reductase (MTHFR); and deficiencies of protein C, protein S, and antithrombin. The following frequencies (patients v controls), odds ratios (ORs), and confidence intervals (CIs) of single risk factors were found: Lp(a) G30 mg/dL (26.4% v 4.7%; OR/CI, 7.2/3.8 to 13.8; P F .0001), FV G1691A (20.2% v 4%; OR/CI, 6/2.97 to 12.1; P F .0001), protein C deficiency (6% v 0.67%; OR/CI, 9.5/2 to 44.6; P 5 .001), PT G20210A (6% v 1.3%; OR/CI, 4.7/1.4 to 15.6; P 5 .01), and the MTHFR TT677 genotype (23.6% v 10.4%; OR/CI, 2.4/1.53 to 4.5; P F .0001). A combination of the heterozygous FV G1691A mutation with increased Lp(a) (n 5 11) or the MTHFR TT677 genotype (n 5 5) was found in 10.8% of cases, but only 0.3% of controls (OR/CI, 35.75/4.7 to 272; P F .0001). Increased Lp (a) levels, the FV G1691A mutation, protein C deficiency, the prothrombin G20210A variant, and the MTHFR TT677 are important risk factors for spontaneous ischemic stroke in childhood. r 1999 by The American Society of Hematology.

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Prothrombotic Genetic Risk Factors in Young Survivors of Myocardial Infarction

It has long been thought that an individual thrombotic tendency increases the risk of myocardial infarction, especially in young adults. Several ‘‘prothrombotic’’ genetic factors that may influence the individual thrombotic risk have been identified. To investigate the association between the risk of myocardial infarction at a young age and genetic factors thought to be associated with an incre...

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Factor V Leiden: a genetic risk factor for thrombotic microangiopathy in patients with normal von Willebrand factor–cleaving protease activity

Thrombotic microangiopathy (TM) is associated with abnormalities of von Willebrand factor–cleaving protease (VWCP) and other hemostatic factors. This study hypothesized that TM patients might have genetically determined thrombotic risk factors that predispose them to aberrant microvascular thrombosis. DNA samples from 30 white and 12 African American adult TM patients were analyzed for genetic ...

Evaluation of Clinical Trials

In a number of important clinical issues such as evaluation of the efficacy or effectiveness of therapeutic or preventive interventions as well as for comparing the harms of interventions, randomized controlled trials (RCTs) provide the highest levels of evidence, either directly or indirectly. It is obvious that critical appraisal of these studies to assess their validity and precision is of p...

CLINICAL OBSERVATIONS, INTERVENTIONS, AND THERAPEUTIC TRIALS Factor V Leiden mutation, prothrombin gene mutation, and deficiencies in coagulation inhibitors associated with Budd-Chiari syndrome and portal vein thrombosis: results of a case-control study

In a collaborative multicenter case-control study, we investigated the effect of factor V Leiden mutation, prothrombin gene mutation, and inherited deficiencies of protein C, protein S, and antithrombin on the risk of Budd-Chiari syndrome (BCS) and portal vein thrombosis (PVT). We compared 43 BCS patients and 92 PVT patients with 474 population-based controls. The relative risk of BCS was 11.3 ...

Twelve-single nucleotide polymorphism genetic risk score identifies individuals at increased risk for future atrial fibrillation and stroke.

BACKGROUND AND PURPOSE Atrial fibrillation (AF) is prevalent and there is a clinical need for biomarkers to identify individuals at higher risk for AF. Fixed throughout a life course and assayable early in life, genetic biomarkers may meet this need. Here, we investigate whether multiple single nucleotide polymorphisms together as an AF genetic risk score (AF-GRS) can improve prediction of one'...
